I suggested that he may be an emerging Schizotypal Personality Disorder. Although personality disorders, in general, aren't diagnosed until age 18 because the personality is still evolving, an evaluation will be helpful to determine his profile of strengths and weaknesses. If he is experiencing delusional or hallucinatory behaviors, a medical management plan can be implemented.
Essentially, my initial goal would be to stop the deterioration from becoming full-on schizophrenia.
Areas of focus would be on developing social and coping skills and to educate parents about ways to reduce his anxiety.
This family is in very serious distress. It may help them to learn more about Schizotypal Personality Disorder by doing an extensive online search focusing on only those legitimate medical/psychological sources.
